WHAT SERVICES DO WE PROVIDE?

Our multidisciplinary team, supported by our administrative team, provides primary health care services for our registered clients. We also provide a wide range of health promotion and disease prevention programs for our community.

Physician Services

Family physicians’ practice at the Centre, providing care for our rostered clients in a team concept, collaborative approach to primary health care. An appointment is necessary to see a physician. In order to provide continuity of care, you may choose to see the same provider. If that provider is unavailable you may be seen by another member of the health care team. If you have no family physician, and wish to become a client at the Centre, please ask for a client application form at reception.

Nurse Practitioner Services

A nurse practitioner (NP) is a registered nurse with advanced university education who provides personalized, quality health care. Ontario nurse practitioners provide a full range of health care services to individuals, families and communities.

Nurse Practitioners’ practice at the Centre, providing care for our rostered clients in a team concept, collaborative approach to primary health care. An appointment is necessary to see a Nurse Practitioner. In order to provide continuity of care, you may choose to see the same provider.  If that provider is unavailable you may be seen by another member of the health care team. If you have no family physician, and wish to become a client at the Centre, please ask for a client application at reception.

NPs work in partnership with physicians, nurses and other health care professionals to keep people healthy.

Registered Nurse/ Registered Practical Nurse Services

The Registered Nurses and Registered Practical Nurses anchors our Clinical Team by supporting both the physicians and nurse practitioners in their practice, as well as coordinating referrals, advocating on behalf of clients, and providing individual client education and nursing interventions. Nurses are also involved with many health promotion and prevention programs.

Dietitian Services

You can refer yourself to counselling relating to our dietitian for individual, heart health, weight management or any other nutritional information you may require. Any community member can use this service simply by calling and booking an appointment.

Social Work/Counselling

A social worker is available by appointment. You can refer yourself by calling the Centre. The counsellor will return your call to assess need and urgency of your request.

The following are the priority groups for registering CKCHC primary health care clients:

We are designed with the community in mind and for that reason we serve persons of all ages who are economically and/or socially disadvantaged with a focus on:

  • Mental health and addictions
  • Persons that face barriers because of race, language, culture and sexual orientation locally representing:
    • Indigenous peoples
    • Francophone
    • Low German persons
    • Sexually and gender diverse persons
    • Temporary foreign workers, immigrants, refugees and those challenged with literacy
  • Persons who are homeless/under-housed or at risk for homelessness

Applicants MUST be a resident of Chatham-Kent or the surrounding area.

Applicants will be prioritized on a waiting list for intake; priority is given to those without primary care. We will be registering priority group applicants without primary care first. Acceptance to our practice (Physician / Nurse Practitioner) will be made on the basis of available openings in the teams’ caseload.

Once you become a registered client, you will have an appointment with a Nurse to go over your medical history.  You will then be assigned to a Physician or Nurse Practitioner.   At that time, we will explain how the Centre operates and how to access CKCHC’s programs and health care providers.

Once you are registered with our centre, you will be able to receive primary health care services delivered by a team of Physicians, Nurses, Social Workers, Chiropodist, Dietitians, Addictions Counsellor, Health Promoters, Occupational Therapist and many other health care professionals.

Please be aware that it may take several months for us to consider your application and we would like you to contact us should your health status or personal information change while you are on the waiting list. We appreciate your patience as we go through this process.
