The First Five Program provides preventative and health promotion services to children aged 0-5 who do not have a primary care doctor or nurse practitioner.

Eligibility:

  • The child is 0-5 years old; AND
  • Does not have a family doctor or nurse practitioner
  • No health card is needed

The First Five Nurse Practitioner and Nurse can provide the following services:

  • Well baby/well child visits
  • Immunizations
  • Developmental screening
  • Nutrition support
  • Assistance with navigating the health care system
  • Referral requests
  • Care for acute illnesses (i.e. infections, coughs, colds, vomiting, rashes, etc)

We do not provide:

  • Care for children 6 years of age or older
  • Care for parents/caregivers
  • Obstetrical/prenatal care

This program will be offered at our Chatham, Wallaceburg and Walpole Island sites. Outreach clinics will be available at satellite locations as needed. Translation services are available for non-English speaking families.

Community partners, parents and/or caregivers of children can refer to the program.

NOTE: We do not accept referrals for current pregnancies/unborn infants. Please send us a referral after your child is born.
