How to Become a Client

If you do not have a primary health care provider, and you fall into one of the priority populations we serve, we welcome you to consider applying to become a registered client.

Our centres assist the following community members:

Persons of all ages who are economically and/or socially disadvantaged with a focus on:
– Mental health and addictions
– Persons that face barriers such as race, language, culture and sexual orientation
– Persons who are homeless/under-housed or at risk for homelessness

Once you are registered with our centre, you will be able to receive primary health care services delivered by a team of Physicians, Nurses, Social Workers, Chiropodist, Dietitians, Addictions Counsellor, Health Promoters, Occupational Therapist and many other health care professionals.

In order to become a registered client, you must complete an application form available at any of our three locations or online.  Priority will be given to people who do not have a family physician and who may have difficulty obtaining health services.

Once you become a registered client, you will have an appointment with a Nurse to go over your medical history.  You will then be assigned to a Physician or Nurse Practitioner.
