CHC Model of Care
The CHC Model of Care focuses on five service areas:

The CHC Model of Care is:

3. client and community centred
4. interprofessional
5. integrated
6. community governed
7. inclusive of the social determinants of health
8. and grounded in a community development approach
CHCs offer a range of comprehensive primary health care and health promotion programs in diverse communities across Ontario.
